When You Should Consult an Attorney

- You have questions about child support or spousal support calculations

Free and Low-Cost Legal Help in California
- California Courts Self-Help Center: Most courthouses have a free self-help center that can answer basic questions about court forms
- Legal Aid: If you qualify based on income, legal aid organizations provide free legal help
- Lawyer Referral Services: Your local bar association can refer you to an attorney for a low-cost consultation
Accuracy Disclaimer
While we strive to keep our tool up to date with the current version of the FL-150 form, court forms can change. It is your responsibility to verify that the completed form meets your court's current requirements. We recommend checking with your local court clerk if you have any concerns.
Declaration Under Penalty of Perjury
The FL-150 form requires you to sign under penalty of perjury that the information you provide is true and correct. This is a legal obligation. Providing false information on a court form is a crime. Make sure all information you enter is accurate and complete to the best of your knowledge.
