DivorceFormHelper ("we," "us," "our") provides a web-based document preparation tool that helps users fill out the California FL-150 Income and Expense Declaration form. We are not a law firm and do not provide legal advice, legal representation, or legal services of any kind.
2. Not Legal Advice
The information, tooltips, and guidance provided by this tool are for informational purposes only. They explain what each form field is asking for — they do not tell you what to enter. You are responsible for the accuracy and completeness of all information you provide. If you need legal advice, consult a licensed California family law attorney.
3. User Responsibility
You are solely responsible for:

4. No Guarantee
We do not guarantee that the court will accept your completed form, that the form is appropriate for your specific situation, or that using our tool will produce a particular legal outcome. Court requirements may vary by county and may change over time.
5. Payment and Refunds
The fee for using our service is a one-time charge. Because the PDF is generated and downloaded immediately, refunds are generally not available after download. If you experience a technical issue that prevents you from receiving your completed form, contact us and we will work to resolve the issue.
6. Limitation of Liability
To the maximum extent permitted by law, DivorceFormHelper shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses resulting from your use of the service.
7. Intellectual Property
The FL-150 form is a public document created by the Judicial Council of California. Our website design, code, and content (excluding the form itself) are our intellectual property. You may not copy, modify, or distribute our website or its code without permission.
8. Changes to Terms
We reserve the right to modify these terms at any time. Changes will be posted on this page. Your continued use of the service after changes are posted constitutes acceptance of the modified terms.
9. Governing Law
These terms are governed by the laws of the State of California. Any disputes arising from these terms or your use of the service shall be resolved in the courts of Orange County, California.
